One has to have in place all the checks and balances for execution of perfection. In addition one has to be diligent , vigilant in every step of execution and have all the intent to do a perfect job or expectation of a perfect job. When a imperfection occurs then there has to a plan to review the root cause and implement changes and move on with accepting perfection. I have found out that people who makes mistakes feel bad themselves so we have to stress perfection and not chastise people, make them aware of the imperfection and the lessons learned.
